Enrollment checklist

A parent or guardian confirms that the student can attend CTC programming.

Families choose whether CTC may use photos or videos of their student.

Emergency contact, allergies, medical notes, and permission to respond in an emergency.

Clear expectations for students, volunteers, speakers, and adults in the room.

A signed record tied to the student’s enrollment. Final view-only forms will live here when approved.

We use contact details only to answer requests, coordinate programs, and send enrollment information. We do not sell personal information.
